Any student wanting to participate on an Athletic team during the 2016-2017 school year must complete and return a physical packet, acknowledgement form that the student and the parent have read the Student-Athlete Handbook, which includes the OCPS Transportation SU246 form.
If you are a sophomore, junior or senior and a new student to University High School, you will also need to complete, sign, and have NOTARIZED the GA4 form.
If you are a home school, charter school, or FLVS student, please click on the appropriate document on the right for additional paperwork that needs to be completed and submitted to the Athletic Director before participation.
If you are a foreign exchange, international, or immigrant student, please see the appropriate document for additional paperwork needed from you to participate in the athletic program.
If you are participating in conditioning, open gym, or other athletic events outside of the FHSAA season, a Waiver for Off-Season Sports Activities must be completed and returned to the Head Coach for that sport.
Physical packets must be turned into the Athletic Training Room, located in the gym, during lunch to our Athletic Training Staff. All other forms must be submitted to Coach Barnard, Athletic Director, in Office 3-117 before being cleared to participate in any athletic program.
